The therapeutic advisor, a powerful analysis engine.

An advisor is an analysis software. It displays potential problems related to the information specified in the patient’s record. In addition to detecting the interactions, the advisor analyzes the patient’s complete profile according to 18 criteria and transmits relevant alerts when justified. (The advisor communicates with any other software qualified to provide drug profiles through an interface. Vigilance Santé supplies software developers with all the necessary technical information in order to create such an interface).

The alert system is a simple one, based on the appropriate level of clinical intervention required to handle the problem effectively.

The software brings security to all healthcare professional users, whether they be consultant physicians or pharmacists.

Within the profile, the therapeutic advisor can identify 18 types of problems presented below:

DRUG + DRUG INTERACTION

Two medications in profile could cause a clinically significant drug interaction.
Interactions monographs are available to give more information on the interaction and its management

DIAGNOSTIC + DRUG WARNING

A medication in profile should be avoided or used with caution in the presence of a medical condition also present in the profile.
The DUR recognizes default diagnoses for more than 1000 drugs and can signal contraindications even if those diagnoses have not been specified in the profile.

ALLERGY / CROSS-ALLERGY

A medication in profile has previously caused an allergic reaction to this patient or has a cross-allergy potential with a medication that has previously caused an allergic reaction to this patient.

This alert type also covers inactive ingredients recognized as allergenic (e.g. latex, peanut derivatives, sulfites, tartrazine) and medication present as traces (e.g. antibiotics in many vaccines). The message conveys the “certainty level” of the allergy, as specified in the profiles as 1) a confirmed previous allergic reaction to product, 2) a confirmed previous reaction to product or 3) an unconfirmed previous allergic reaction to product.

INTOLERANCE

A medication in profile has previously caused an intolerance reaction to this patient. This alert type also covers inactive ingredients such as gluten and lactose.

ATYPICAL ASSOCIATION

Some products in the profile are not usually combined in therapy. This alert type is used for non-recommended combinations that are neither “true” therapeutic duplications nor drug interactions. It also covers the situation when a problem is caused by the combination of more than two active ingredients.

ADDITIVE TOXICITY

Many medications in the profile share a similar side effect potential. Targeted side effects are QT interval prolongation (arrhythmia), drowsiness, anticholinergic effects, serotoninergic syndrome, and hyperkalemia. Each medication may bring a toxicity risk. When the bearable maximal threshold is reached by the contribution of several medications known to produce potential toxicity, an alert is generated. The warning threshold for each type of toxicity is modulated according to the patient’s age and gender. The tolerance thresholds are higher in a hospital environment than in community-based setting.

AGE-RELATED DRUG WARNING

A medication in profile should be avoided or used with caution based on patient age. This alert type covers both pediatric and geriatric contraindications. The alert messages give a justification for the warning and the severity levels vary for each medication.

RENAL FAILURE DOSAGE ADJUSTMENT

The dosage of a medication in profile must be adjusted for the renal failure also noted in the profile.

Renal failure can be directly identified in the profile or extrapolated from the creatinine clearance. The creatinine clearance itself can be calculated if the serum creatinine level is provided in the profile along with patient gender and weight. The alert will be triggered even if the creatinine clearance is not specified. The creatinine clearance is not required for this alert to be generated.

If the creatinine clearance is provided or was calculated by the analysis engine, the alert message will identify the expected dosage adjustment for this specific clearance.

DIALYSIS + DRUG IMPACT

An active ingredient in profile requires a dosage adjustment for dialyzed patients.

DOSAGE / CUMULATIVE DOSAGE

There is a potential problem with the dosage of a medication in profile. The daily dosage can be extrapolated from the quantity dispensed and duration of treatment. The validation covers the daily dose, unitary dose (when provided) as well as cumulative dose and will alert for doses too high or too low. The alert messages contain the calculated doses and the expected thresholds.

DUPLICATION OF MEDICATION

A medication is present in more than one product in profile.

DUPLICATION IN THERAPEUTIC CLASS

Multiple agents from the same pharmacological class are present in the profile.

LABORATORY RESULT + DRUG IMPACT

This profile shows abnormal laboratory test results wich may be related to active medication.

LIFESTYLE + DRUG INTERACTION

A medication in profile should be avoided or used with caution in the presence of a patient lifestyle preference or limitation. This will signal products that should be avoided in patients that give blood, eat grapefruit (or drink grapefruit juice), crush their medications, have a difficulty to swallow their medications or participate in high-level sport competitions.

A medication in profile requires a dosage adjustment for patients who are smoking.

A medication in profile should be avoided or used with caution by contact lens wearers.

MEDICATION TO AVOID

A medication in profile should be avoided. Based on medications signaled in the profile as having to be avoided (contains alcohol, specific medication or pharmacological class, medications previously shown to be ineffective).

PREGNANCY + DRUG IMPACT

A medication in profile should be avoided or used with caution by pregnant women.

BREASTFEEDING + DRUG IMPACT

A medication in profile should be avoided or used with caution by breastfeeding mothers.

COLOSTOMY / ILEOSTOMY

A medication in profile should be avoided or used with caution in the presence of a colostomy or ileostomy.
